Patient Consent.

Before receiving treatment at Strength & Pilates Physiotherapy, all patients are asked to provide informed consent. The following outlines what this means and what you are agreeing to.

💡

Your consent is voluntary and can be withdrawn at any time. Once you have given consent, you may withdraw it at any point during your treatment without affecting your right to receive care.

Your condition and treatment options will be discussed so that you are appropriately informed and, together with your physiotherapist, can make decisions relating to your treatment.

You are entitled to refuse any form of treatment and are encouraged to communicate any concerns with your physiotherapist. Foreseeable risks will be discussed with you prior to administering any treatment.

By signing your intake form (or by attending your appointment having reviewed this page), you give your voluntary consent for the administration of treatment deemed appropriate by your treating therapist. You understand that the primary goals of your treatment are to help reduce pain and improve mobility, strength, endurance, function, and quality of life.

Personal Information.

In order to obtain a clear picture of your injury and its impact on your daily activities and function, your physiotherapist may ask questions of a personal nature. The more relevant information you provide, the better we can tailor a treatment plan specific to your needs.

Our staff adhere to the Privacy Act 1988 (Cth) and the Australian Privacy Principles. We recognize the importance of a trusting relationship for sharing personal information and treat all information with the utmost respect and confidentiality.

Your information will not be disclosed to any third party without your consent, except where required by law or for direct treatment purposes (e.g. correspondence with your referring GP).

🤖

AI-assisted note-taking: We may use AI-assisted software to securely capture and manage session notes. For full details on data handling and security, please visit patientnotes.app/privacy-and-compliance. If you have concerns, please speak with your physiotherapist before your session.

Physical Contact.

It is likely that physical contact will be necessary during the course of examination, assessment, and treatment. Your physiotherapist will always explain what they are doing and why before proceeding.

Please inform your physiotherapist if anything can be done to assist your comfort, or if you have any concerns. If you feel uncomfortable at any time, please inform your physiotherapist and any physical contact can cease immediately.

Children & Minors.

For the treatment of a minor, the consent form must be signed by a custodian (parent or legal guardian).

The presence of a parent or caregiver is requested for anyone under the age of 16 years receiving treatment. For patients aged 15 and over, written parental consent is sufficient if a parent or guardian cannot attend in person.

Treatment Risks.

We believe in fully informed consent. Below is a clear summary of the treatment modalities we may use and their associated risk factors. All risks are small but possible, and foreseeable risks will always be discussed with you before any treatment begins.

Treatments may include modalities such as heat, ice, therapeutic taping, TENS, interferential current, shockwave therapy, and electrical muscular stimulation. Other options include acupuncture or dry needling, involving the insertion of single-use, sterile, disposable needles through the skin into the underlying muscles.
